Payment & Service Terms

Accepted Payment Methods

 

 

​We accept the following forms of payment for all services:

  • Major Credit & Debit Cards

  • Business Invoicing (approved clients only)

  • Digital Payments (as provided upon request)

Service Billing

Pricing is determined based on:

  • Service type

  • Location and travel requirements

  • Number of individuals or collections

  • Scheduling needs (standard vs. urgent)

A clear quote will be provided prior to confirming your service.

Deposits & Scheduling

A deposit may be required to secure your appointment, particularly for:

  • Same-day or urgent service requests

  • New clients

  • Large or multi-person collections

Appointments are not guaranteed until confirmed

Payment Timing

  • Payment is due prior to or at the time of service unless otherwise arranged

  • Net payment terms may be available for approved employers, TPAs, and partner organizations

Important Information

Attest Collective operates as a direct service provider

  • Services are not billed through insurance

  • Clients are responsible for all service-related fees

 

Late Arrival / Wait Time Fee

​A grace period of  20 minutes is allowed. Delays beyond this window may be considered a missed appointment or subject to additional wait-time fees.

Primary service area includes Central New Jersey. Expanded service areas are available upon request and subject to travel fees.

If you require a customized billing arrangement, our team is happy to discuss options.

 

Attest Collective does not guarantee test outcomes, turnaround times, or specific results, as these are determined by the contracted laboratory and external processing factors

Chargebacks & Payment Disputes


Clients agree not to initiate chargebacks for services that have been scheduled, performed, or made available. Any billing concerns must be addressed directly with Attest Collective prior to initiating a dispute. Unauthorized chargebacks may result in additional administrative fees or denial of future services.
