Attest Collective provides mobile specimen collection and compliance support for employers, laboratories, and third-party administrators across Central New Jersey.  

 

The following policies outline our service scope, scheduling practices, privacy standards, and collection procedures to ensure professional, reliable service for every client and partner.

Mobile Service Policy

 

Attest Collective provides mobile compliance services and on-site specimen collection at client locations. 

Appointments are scheduled in advance; however, short-notice and same-day service may be available depending on scheduling and location. 

Travel outside the primary Central New Jersey service area may require additional travel fees.

 

Clients are responsible for ensuring the collection location provides a safe and appropriate environment for specimen collection when required.

Attest Collective provides professional specimen collection and identity verification services only.

 

Laboratory testing, analysis, and result reporting are performed by certified laboratories and Medical Review Officers (MROs) when applicable.

 

Attest Collective does not interpret laboratory results or make employment, legal, or medical determinations related to testing outcomes.

 

Attest Collective does not receive, release, or interpret laboratory results.

All results are reported directly by the contracted laboratory or Medical Review Officer (MRO) to the appropriate authorized parties.

 

All collections are conducted in accordance with established chain-of-custody procedures and industry collection standards.

 

 

 

 

Privacy & Data Protection Policy

Attest Collective respects the privacy and confidentiality of all individuals and organizations we serve.

Information collected during the course of services may include personally identifiable information necessary to perform specimen collections, employment verification, background screening coordination, or identity verification services.

Client information is handled with discretion and shared only with authorized laboratories, Medical Review Officers, third-party administrators, or service partners required to complete the requested service.

Attest Collective does not sell or distribute personal information to outside parties for marketing purposes.

Data Sharing

  • Customer data is not shared with 3rd parties for promotional or marketing purposes.

  • Mobile opt-in and consent are never shared with anyone for any purpose. Any information sharing that may be mentioned elsewhere in this policy excludes mobile opt-in data.

 

 

Messaging Terms and Conditions

  1. The messaging program consists of general conversational messaging to answer questions and provide support to customers.

  2. You can cancel the SMS service at any time. Just text "STOP" to the phone number from which you received messages. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

  3. If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at support@attestcollective.com.

  4. Carriers are not liable for delayed or undelivered messages.

  5. As always, message and data rates may apply for any messages sent to you from us and to us from you. Message frequency will vary based on communication needs. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.

Appointment & Cancellation Policy

Attest Collective schedules mobile services in advance whenever possible; however, short-notice and same-day appointments may be available depending on availability and location.

 

Clients are asked to provide reasonable notice if an appointment must be rescheduled or canceled.

A reasonable grace period is allowed for scheduled appointments. Delays beyond this window may be considered a missed appointment and subject to applicable fees.

Same-day cancellations, missed appointments, or situations where the collector arrives and the service cannot be completed may be subject to a service fee.

Travel fees may apply for locations outside the standard Central New Jersey service area.

Attest Collective reserves the right to decline services that cannot be performed safely, ethically, or in accordance with applicable collection standards.

 

​Services may be discontinued or refused if a donor is non-compliant, the environment is unsafe, or proper identification and documentation cannot be verified.

Refund & payment policy

Payment is due at the time of service unless prior arrangements have been made.

 

Due to the nature of mobile and on-site services, fees are generally non-refundable once an appointment has been scheduled and confirmed.

 

Refunds may be considered at the sole discretion of Attest Collective.

Deposits, when required, are applied toward the total service cost and may be non-refundable depending on scheduling commitments.

 

 

Consent & Authorization

​​By requesting services, clients acknowledge that proper consent will be obtained from all individuals being tested or verified.

Attest Collective requires appropriate identification and authorization prior to performing any specimen collection or verification service.

 

For employer-requested services, the client is responsible for ensuring compliance with applicable workplace policies and consent requirements.

 

Services will not be performed without appropriate consent and required documentation.
